3. Patient-Provider Relationship

Use of this website or sending an inquiry through a contact form does not establish a doctor-patient relationship with Peak Balance Mental Health or our Mental Health Provider. A formal relationship is only established after an initial psychiatric evaluation and the signing of a formal treatment agreement.

4. Appointment & Cancellation Policy

By scheduling an appointment at Peak Balance Mental Health, you agree to our clinic policies:

• Cancellations: We require [e.g., 24 or 48] hours' notice for cancellations. Failure to provide notice may result in a cancellation fee.
• Spravato Sessions: Specialized treatments require specific arrival times and a designated driver. Failure to meet safety requirements will result in a rescheduled session.
